Why stress is showing up in your pelvic floor

When people think about stress, they usually think about headaches, poor sleep, or feeling mentally overwhelmed. What many people don't realize is that stress can also show up physically in the pelvic floor.

I see this all the time.

Sometimes people come in with pelvic pain, urgency, constipation, pressure, leaking, or discomfort during intimacy — and they've already tried stretching, strengthening, or "fixing" the muscles themselves. But no one has explained that the nervous system may be playing a role too.

When your body spends long periods of time in stress mode, your muscles often stay in a guarded, tense, protective state without you even realizing it. That includes the pelvic floor.

This doesn't mean symptoms are "all in your head."
It means your body is responding exactly the way human bodies are designed to respond under stress.

Honestly, this is something I've been paying more attention to in my own life too. Lately, I've been trying to avoid checking Instagram first thing in the morning, not looking at texts for the first hour after waking up, playing simple brain games instead, and prioritizing movement throughout the day by aiming for 10k steps.

Not because these habits magically "fix" stress — but because I've realized how much our nervous systems absorb from the pace and stimulation of modern life.

And if our bodies rarely get moments of regulation, slowing down, movement, or quiet, it makes sense that tension can start showing up physically too.

A few signs your nervous system may be contributing to pelvic floor symptoms:

  • You feel like you can never fully relax
  • You clench your jaw, shoulders, or stomach often
  • You rush through the day without pauses
  • You frequently feel "on edge"
  • Your symptoms flare during stressful periods

This is one reason pelvic health care should look beyond exercises alone.

Sometimes recovery also involves:

  • learning how your body holds tension
  • improving breathing patterns
  • changing daily habits
  • creating moments of nervous system regulation
  • helping your body feel safe enough to relax again

Small changes can make a bigger difference than people realize.

And often, understanding your body is the first step toward healing it.
