The symptom nobody brings up at their annual

By Erica Bermudez, OTR/L — pelvic floor occupational therapist

There's a symptom my patients almost never lead with. It comes out in session three, quietly, like a confession: "also... sex hurts."

If that's you, I want you to know two things.

First, you're not broken, and you're not alone. Pain with sex shows up after babies, after menopause, after infections, after years of clenching through stress — and sometimes with no obvious trigger at all. The muscles of your pelvic floor can guard and tighten just like a neck that won't turn after you slept on it wrong.

Second — and I need you to internalize this one — it is not something you owe anyone. Not something to push through, wine your way through, or schedule around. Pain is information, not a personality flaw.

This is where being an OT matters. I don't just treat the muscle. We look at the stress, the guarding, the story your body has learned to tell — because pelvic pain is almost never just mechanical.

No pitch this week. I just know how many of you are sitting on this one silently.

If this hit home, reach out — I'll point you in the right direction, even if that direction isn't me.
